Maplewood Y Artisan Craft Show - Artisan Registration Form

SATURDAY, OCTOBER 17, 2015, 9:00am-3:00pm

To apply to the Fifth Annual Maplewood Y Artisan Craft Show, please fill out the following registration form by June 31, 2015. Once we receive your application, we will send you a confirmation email.  To complete the application process you will need to send 3 digital pictures, representative of your current work, to by June 31, 2015.  Once we receive your 3 digital photos, we will send you another confirmation email. 
The show is a blind juried show. Your 3 digital photos will be posted, with an identification number only, on the show’s Facebook Page in a photo album. ( Three jurors will judge your work and rate it within the “Judging Classification” you chose below. The show will be filled with the highest rated artisans based on a percentage distribution from each classification.
You will receive an email no later than July 15, 2015, letting you know if you have been accepted for the show or have been placed on the waiting list. This email will give you instructions on how to pay the booth fee. All booths are 8 feet x 8 feet and are $45. The booth fee, once paid, will be non-refundable.  A donation will be required from each artisan for the raffle.
If you are intending to share a booth with a fellow artisan, all of you need go through the application process. There is a section in the application to let us know that you will be sharing a booth.
